Why is it so difficult to stand out from the crowd?

Many clients have asked me this. I often see the following common problems:
Lack of target group analysis / ICP definition – they use the same generic messaging to everyone, no segmentation.
Poor / unclear Value Proposition – not clear what they offer to whom and why (why should they care), often a "packaging" problem.
Lack of content strategy (based on the above) – their content is a bit all over the place, feels random, generic, does not feel relevant to or address customers' problems.
Brand identity – they look the same as everyone else in their industry or their profile doesn't fit with their target audiences and market positioning.
Channel selection – they are not using the right channels to reach their target audiences, or they're using the wrong format/tactic for the channel.
